Patient Access now available to Crown Dale patients!

  We have now introduced Patient Access to our patients. This allows you to book GP appointments online, request prescriptions and view parts of your medical record.

Please see a member of the reception staff who will register you for this service and give you log-in details. The process is quick and simple so why not register today!
 
This service is currently available for adult (age 16 and over) patients only.

Alternatively you can complete the questionnaire in the surgery - please ask at the reception desk or the healthcare professional you see.

Prescriptions

* Please note the existing method of ordering repeat prescriptions online will cease from 1st April 2015. To continue ordering repeat prescriptions online please register for Patient Access (see details opposite)

General Reminder: Please allow 3 working days for your prescriptions to be processed.

Out of Hours

Our doctors are part of SELDOC (South East London doctors on Call), a doctors' co-operative where GPs take turns to do night and weekend duties. For urgent advice when the surgery is closed please telephone SELDOC (020 8693 9066). In the case of an acute collapse or dire emergency, please dial 999.

Sexual Health Clinic 

The practice also runs a walk in Sexual Health Clinic on Wednesday evenings between 6.30-8.00pm and on Saturday mornings between 9.30-11.45am. 

Test Results

Please call the the results line direct on 020 8655 7856 or the main number on 020 8670 2414 and press option 2 for results. The results line is only available between 2.00 - 4.00pm, Monday to Friday.

Repeat Prescriptions

We cannot take requests for repeat prescriptions over the telephone. You can, however, now request your repeat prescription on-line via our website.

Telephone Clinics

We now offer telephone clinics every morning with one of our GP's.  The clinics are for patients who would like advice and who feel that a face to face appointment is not necessary.  Please ask the receptionist for more informaton. 

Phoning the Practice

Please be aware that we receive a large amount of calls between 8.00am and 9.30am. Should your call not be about an appointment we would recommend that you telephone in after 10.30am.

Have you received a letter from NHS England?

If you receive a letter in the post from NHS England, it is very important that you respond.  The letter will ask you to complete a form to confirm that your address is correct.

If you do not respond, NHS England will remove you from our patient list within a period of 6 months and you will then need to re-register with us.

Research practice  

Crown Dale Medical Centre is a research practice.  We are regularly involved in research activity.  We would always obtain informed patient consent before participating in any form of research.

If you have any concerns please ask to speak to the practice manager.

 

 

GP Training

You may be offered an appointment with a GP Registrar, who is a fully qualified doctor. These doctors spend six months to one year in our practice before finding their own practice or returning to hospital work.

Student doctors, who are not yet fully qualified, occasionally sit in during consultations to gain experience. You will be informed when a student is in the surgery.

Please advise the receptionists if you would prefer a student not to be present during your consultation.

To help in teaching, we sometimes use audio and video recordings of consultations, although only with the patients' permission. If you have any objection please tell the doctor.
