Patient Access now available to Crown Dale patients!

  We have now introduced Patient Access to our patients. This allows you to book GP appointments online, request prescriptions and view parts of your medical record.

Please see a member of the reception staff who will register you for this service and give you log-in details. The process is quick and simple so why not register today!
This service is currently available for adult (age 16 and over) patients only.

Out of Hours

Our doctors are part of SELDOC (South East London doctors on Call), a doctors' co-operative where GPs take turns to do night and weekend duties. For urgent advice when the surgery is closed please telephone SELDOC (020 8693 9066). In the case of an acute collapse or dire emergency, please dial 999.

Sexual Health Clinic 

The practice also runs a walk in Sexual Health Clinic on Wednesday evenings between 6.30-8.00pm and on Saturday mornings between 9.30-11.45am. 

Test Results

Please call the the results line direct on 020 8655 7856 or the main number on 020 8670 2414 and press option 2 for results. The results line is only available between 2.00 - 4.00pm, Monday to Friday.

Repeat Prescriptions

We cannot take requests for repeat prescriptions over the telephone. You can, however, now request your repeat prescription on-line via our website.

Telephone Clinics

We now offer telephone clinics every morning with one of our GP's.  The clinics are for patients who would like advice and who feel that a face to face appointment is not necessary.  Please ask the receptionist for more informaton. 

Phoning the Practice

Please be aware that we receive a large amount of calls between 8.00am and 9.30am. Should your call not be about an appointment we would recommend that you telephone in after 10.30am.

Have you received a letter from NHS England?

If you receive a letter in the post from NHS England, it is very important that you respond.  The letter will ask you to complete a form to confirm that your address is correct.

If you do not respond, NHS England will remove you from our patient list within a period of 6 months and you will then need to re-register with us.

Research practice  

Crown Dale Medical Centre is a research practice.  We are regularly involved in research activity.  We would always obtain informed patient consent before participating in any form of research.

If you have any concerns please ask to speak to the practice manager.




NHS Referrals

Most of our referrals are made via Choose & Book, this is a system which gives patients choice about the hospital they are referred to and the timing of their appointments. When we process the referral you will be sent a letter from us with either a booked appointment or a contact number and details to enable you to book your own appointment.
Please tell the GP in your consultation which hospital you would like to be referred to from the following list:
Kings College Hospital - Camberwell
Mayday Hospital - Croydon
St Georges Hospital - Tooting
Guys Hospital – London Bridge
St Thomas Hospital – Waterloo/Westminster
You may be able to choose alternative hospitals, if you have a particular preference, please let us know.
If you have been referred to a hospital by your GP and have not heard about your appointment please ring the Secretarial Team on 020 8670 2414 (select option 3) if you would like to enquire about the progress of your referral. Please allow the following time scales before you ring the Secretarial Team here to follow up your enquiry.
Two Week Wait Referrals – Please allow 3 days
Urgent Referrals – Please allow at least a week
Routine Referrals – Please allow at least 2-4 weeks
Private Referrals
If you have requested a private referral, you may wish to book the appointment yourself if you know the doctor or hospital you would like to go to.  If you need a referral letter a member of the secretarial team will contact you by telephone once the referral has been typed and inform you that the letter is ready to be collected from reception.
