Patient Access now available to Crown Dale patients!

  We have now introduced Patient Access to our patients. This allows you to book GP appointments online, request prescriptions and view parts of your medical record.

Please see a member of the reception staff who will register you for this service and give you log-in details. The process is quick and simple so why not register today!
This service is currently available for adult (age 16 and over) patients only.

Alternatively you can complete the questionnaire in the surgery - please ask at the reception desk or the healthcare professional you see.

Out of Hours

Our doctors are part of SELDOC (South East London doctors on Call), a doctors' co-operative where GPs take turns to do night and weekend duties. For urgent advice when the surgery is closed please telephone SELDOC (020 8693 9066). In the case of an acute collapse or dire emergency, please dial 999.

Sexual Health Clinic 

The practice also runs a walk in Sexual Health Clinic on Wednesday evenings between 6.30-8.00pm and on Saturday mornings between 9.30-11.45am. 

Test Results

Please call the the results line direct on 020 8655 7856 or the main number on 020 8670 2414 and press option 2 for results. The results line is only available between 2.00 - 4.00pm, Monday to Friday.

Repeat Prescriptions

We cannot take requests for repeat prescriptions over the telephone. You can, however, now request your repeat prescription on-line via our website.

Telephone Clinics

We now offer telephone clinics every morning with one of our GP's.  The clinics are for patients who would like advice and who feel that a face to face appointment is not necessary.  Please ask the receptionist for more informaton. 

Phoning the Practice

Please be aware that we receive a large amount of calls between 8.00am and 9.30am. Should your call not be about an appointment we would recommend that you telephone in after 10.30am.

Have you received a letter from NHS England?

If you receive a letter in the post from NHS England, it is very important that you respond.  The letter will ask you to complete a form to confirm that your address is correct.

If you do not respond, NHS England will remove you from our patient list within a period of 6 months and you will then need to re-register with us.

Research practice  

Crown Dale Medical Centre is a research practice.  We are regularly involved in research activity.  We would always obtain informed patient consent before participating in any form of research.

If you have any concerns please ask to speak to the practice manager.



How to See a Doctor or Nurse 

calendarFollowing our patient survey at the end of last year and many comments from all of you during consultations with doctors or nurses here, we have decided to make alterations to our daily service and remove the walk-in clinic which we have been operating on Mondays and Thursdays.  Many of you have been unhappy having to turn up on walk-in clinic days and wait some considerable time before being seen by a doctor.  We have listened to what you’ve said.


Please call 020 8670 2414, Monday to Friday to

  • either book an appointment in advance to see a GP or a nurse;
  • or, for an urgent appointment on the same day, a member of the practice team will first take your details and a doctor will then call you back to either give you advice over the telephone or book you an appointment for that day or another day.


Use your health service wisely.  Accident and Emergency at your local hospital are there for just that; an accident or emergency only.  They are there for things like a major injury or a life threatening condition.

Don’t forget that your local pharmacy is often able to help with a large number of minor illnesses, so you may even want to contact them.

For any other emergencies, if it outside the hours that we are open, you should contact the local out-of-hours GP service called SELDOC on 020 8693 6066.

Please be aware that it is not always possible to be seen by your preferred GP. This is because our GP’s all work at varying times across the week and may not be available on the day when you need to see them. We will however endeavour to try and accommodate requests for a particular GP where possible.

We offer telephone consultations for when you feel that there is not a need to see a GP but where telephone advice may be appropriate.

We also offer appointments with our senior practice nurse who can advise patients about minor ailments.

If you can't keep your appointment, please let us know in good time so that it is available for us to offer to another patient. Patients who continuously fail to turn up for their appointments may be asked to find an alternative GP Practice.  

We have a 'Touch Screen' booking in system, so when you arrive, please use this to let us know that you have arrived.
